What Is the McKenzie Method?

The McKenzie Method® is a system of assessment and treatment designed to help clinicians better understand how a person's symptoms respond to specific movements and positions. Rather than assuming that everyone with the same diagnosis needs the same treatment, the McKenzie Method looks at how your individual symptoms behave. During an assessment, we may evaluate how repeated movements, sustained positions, and everyday activities affect your symptoms.
For example, certain movements may:
- Reduce pain
- Increase pain
- Improve movement
- Cause symptoms to move closer to the spine
- Cause symptoms to move farther away from the spine
These responses can help guide an individualized treatment plan.
More Than Just an Exercise Program
The McKenzie Method is sometimes associated with specific exercises, such as repeated bending or extending movements. However, the process begins with a detailed assessment. Not every patient needs the same exercises. A movement that helps one person's back pain may not be appropriate for someone else's symptoms. The goal is to identify movements and positions that may help improve your symptoms and function.
Your treatment plan may include:
- Specific repeated movements

- Postural recommendations

- Manual therapy when appropriate

What Does "Centralization" Mean?
One of the responses we may look for during an MDT assessment is called centralization. Centralization occurs when symptoms that are felt farther away from the spine begin moving closer toward the spine. For example, someone experiencing low back pain that travels into the buttock, thigh, or lower leg may notice that repeated movements cause those symptoms to move closer to the low back. This type of response can provide useful information during the assessment and may help guide treatment.

On the other hand, if symptoms are moving farther away from the spine or becoming progressively worse, that information is also important and may change the approach.
The Importance of Patient Education and Self-Management

One of the primary goals of the McKenzie Method is to help patients become more confident managing their own symptoms. While hands-on treatment can sometimes be helpful, long-term improvement often requires more than passive treatment alone.
We want patients to better understand:
- Which activities may aggravate their symptoms
- Which movements may help reduce symptoms
- How to modify activities when symptoms flare up
- How to gradually return to normal activities and exercise
- How to build confidence in movement again
The goal is not to make patients dependent on ongoing treatment. Instead, we want to provide tools that can help them better manage their symptoms and remain active.
What Conditions Can the McKenzie Method Be Used For?
The McKenzie Method is most commonly associated with the evaluation and treatment of spinal conditions, including:
- Low back pain
- Neck pain
- Pain that radiates into the arm or leg
- Sciatica
- Disc-related symptoms
- Recurrent episodes of back or neck pain
MDT principles can also be applied to certain extremity conditions involving the shoulder, knee, and other joints. Every condition and patient is different, which is why a thorough examination is important before recommending a specific treatment approach.
